DLR/CPS Investigative Assessment
Brief Description
The DLR/CPS Investigative Assessment is designed to give the investigative social worker a user friendly, efficient way to document DLR/CPS investigations. It also provides a way for assessment data to be collected and reported. The assessment contains CA/N findings, risk rating, narrative describing the investigation, substance abuse, overall rating, issues and concerns, and disposition of the investigation.

CREATE A NEW DLR/CPS INVESTIGATIVE ASSESSMENT
1. Click New, select Assessment, select DLR/CPS Investigative Assessment.
2. Type the Referral ID(s) that has the following criteria;
3. The following items are required in order to Save a DLR/CPS Investigative Assessment;
4. Multiple Referrals
A. You can send multiple DLR/CPS Referrals to the same DLR/CPS Investigative Assessment.
B. You must send one Referral to a DLR/CPS Investigative Assessment first before you can send any additional Referrals.
C. If multiple Referrals are selected, the Referrals must be at least 45 days of each other.
D. A worker can assess multiple referrals in a DLR/CPS Invest. Assessment by typing the referral ID, pressing the enter key and then typing additional referral numbers into the Referral Field.

time after it is saved.
DLR/CPS INVESTIGATIVE ASSESSMENT BOILERPLATE

1. Assessment ID: The Assessment ID will be generated by CAMIS once the Assessment is saved.
2. Date of Input: The is the date of actual inputting the DLR/CPS Investigative Assessment into CAMIS.
3. CPS Facility Investigator: This is based on the Worker ID and the security group that the Worker is in.
4. DLR/CPS Supervisor: This is the supervisor of the Worker who has created this Assessment.
5. Office Name: This is based on the Worker information.
6. Facility Name: Based on the Facility Name entered on the Referral.
7. Referral ID: Sent-to or hand typed Referral ID number.
8. Investigation Closure Date: The date that the Supervisor approves the Assessment is the Investigation Closure Date.
GENERAL TAB
1. The Person and Referral information is read only information. This information is generated from the Referral. From the Person section, you can right mouse click on the Person ID, Referral ID, Complaint ID, License ID or Business ID for the corresponding menu.
FINDINGS TAB

Rules for Findings Tab
1. A Founded, Unfounded or Inconclusive Finding must be made for each allegation of CA/N.
2. Findings must be made on subjects and victims.
3. There is only one finding per subject and one finding per victim per CA/N. If more than one CA/N allegation, each CA/N allegation must have a finding.
4. The DLR/CPS Investigator will record findings on all subjects and victims identified in the DLR/CPS Investigative Assessment.
5. DLR/CPS Supervisors may change Findings. Supervisors do this through a secured procedure.
Instructions
1. Check the Founded, Unfounded or Inconclusive box for each CA/N allegation.
2. Basis for Findings text is required for each CA/N allegation that is checked Founded. This is the text that prints in the Founded Subject Notification Letter.
PERSONS/RECORDS TABA. Place your cursor in the Basis for Findings field. You can 1) type right into this field, 2) right mouse click in the Basis for Findings field and click Open text editor or 3) press the F2 key on your keyboard to bring up the text editor. This is helpful if you want to see what you are typing and/or have more than 5 words of text.

B. Type the Basis for Finding in the text editor. When you are finished, click Save to save text entered. Click Cancel to quit the text editor.
C. Do not forget that these columns are resizable. If you need to resize the column to view the text for the entire field, click this cursor
when you are exactly to the right or left of a column heading. The cursor will change to this funny vertical line with arrows cursor, see orange outline in graphic below. Click on the cursor and hold down to resize. This is the same as in Excel.
D. Resizing Basis for Findings is helpful to see more of what you have typed. If there is too much text to resize, place cursor in field and press F2. You will be able to view all of your text in the text editor. If you later change from Founded CA/N to Unfounded or Inconclusive CA/N, the Basis for Findings text will be deleted.
1. Type the text regarding Persons Interviewed and/or Children observed. Be sure to type the Dates, Name and Roles of each Person interviewed.
2. Type the text regarding Records Reviewed. Be sure to type the Dates and Types.
NARRATIVE TAB

1. Click Narrative 1 type text describing facts found during investigation.
2. Click Narrative 2 and/or Narrative 3 to add additional narrative describing the facts found during investigation.

1. Click Support of Findings to type your Narrative in Support of Findings.
2. This narrative is printed in the assessment, but not in the Subject Notification Letter.
1. Characteristics are as follows;
For each Child and Caregiver Characteristic, select a Risk Level and type an Explanation for each Risk Level.
2. Click the Risk drop down arrow to see the following Risk Levels are as follows;
3. After you select a Risk Level, press the Tab key (on your keyboard) to tab to the Explanation field. For every characteristic that has a risk selected, there must be an explanation.
A. Click your cursor in the Explanation field, press F2 (on your keyboard) to open a text editor window. Type the explanation text for that characteristic, and press Save.
1. Click the Substance Abuse Wizard. For each caregiver, choose whether; a determination has been made where it is probable that the use of alcohol or controlled substance is a contributing factor to the alleged abuse or neglect.
2. Substance Abuse Wizard - Determination
A. Click Determination Made drop down arrow. Has a determination been made? Select Yes or No.
B. An Explanation is required if No - Determination Made is selected.

C. Click Next.
D. If there are no subjects or caregivers for whom a determination has been made, then you can click Finish.
3. Substance Abuse Wizard -
Contributing Factors
A. Select Contributing Factor for each Person ID listed. Click Yes or No from the Contributing Factor drop down arrow.
B. Click Next.
4. Substance Abuse Wizard - Referral
A. Click on RCW number below to view policy/law that addresses substance abuse RCW 26.44.170.
B. For each Subject and/or Caregiver, choose whether a referral has been made to a certified chemical dependency specialist or physician for a chemical dependency evaluation.
C. Click Yes or No from the Referral Made drop down arrow.
D. For all Subjects and/or Caregivers for whom a determination has been made, select whether a Referral was made to a certified Chemical Dependency Specialist or Physician.
i.) Select Yes, if a referral has been made.
ii.) Select No, if a referral has not been made.
E. Click Next.
5. Substance Abuse Wizard - Referral Made
A. This is the basis for referral. If you selected Yes on the previous window, you will see the Person ID, name and 4 check boxes. These are indicators of substance abuse which document the basis for referral to a chemical dependency evaluation.
B. Check one of the four choices below. You may enter text in the field to describe how the social worker came to believe substance abuse is an issue to be evaluated. If there was more than one person referred, you must select a basis for referral for each person. Scroll down to see the next person.
Information is obtained from a credible source that the subject of the referral is abuse alcohol or other drugs or is involved in illegal activity involving. (Type description of this activity).
Allegations of substance abuse were made by a credible source in more than one prior referral or substance abuse was identified as an issue in one or more prior CPS investigations. (Type description of allegations).
Substance abuse by the subject was disclosed by the subject or the child during the interview process. (Type description of disclosed substance abuse).
Observations of the subject and the subject's environment indicate a likelihood of substance abuse. (Type description of observations).

C. If you selected No on the previous screen, the check boxes will be ghosted. Click Next.
D. Click Finish.
6. Substance Abuse Wizard - No Referral Made

OVERALL RATING TABA. If you selected No for Referral Made on the Substance Abuse Wizard Referral screen, you will the Person ID and name on this screen. Select a reason why no referral was made with a check in the box next to the reason(s).
Here will be a heading that will list the Person ID, Person Last Name, and First Name.
There is no indication that child abuse or neglect has occurred.
There is no indication that substance abuse is a contributing factor in the alleged child abuse or neglect or to serious risk of child abuse or neglect.
Other. Please describe.
B. Click Finish.

1. Select an Overall Rating of Future Risk by clicking the drop down arrow.

2. Type the Narrative Supporting Overall Rating of Future Risk.
1. Type your text regarding Issues and Concerns noted during DLR/CPS Investigation.
DISPOSITION TAB
1. Select the one Recommended Disposition from the following;
2. Supervisor Approval of Assessment:
A. The worker completes the assessment and checks Assessment Complete.
B. The supervisor gets an email alert re: DLR/CPS Investigative Assessment and the status of Assessment Complete.
C. The supervisor reviews and if approved, checks Supervisor Approval of Assessment.
D. The supervisor can make changes, deselect Assessment Complete if necessary and communicate with the worker what is still needed to complete assessment.
E. When Supervisor Approval of Assessment is checked, the supervisor has until midnight to uncheck Supervisor Approval of Assessment, after that time it is locked permanently.
F. Once Supervisor Approval of Assessment is checked, the Subject Notification Letters can be printed.
- When the supervisor approves the assessment, then an e-mail goes to the licensor to alert that the assessment has been completed and approved.
- An e-mail alert also goes to the assigned case worker if a victim in the assessment is a child related to an open case. (If there is more than one open worker assigned, then the e-mail goes to all assigned.)
3. Assessment Complete
BROWSE DLR/CPS INVESTIGATIVE ASSESSMENTA. If Assessment Complete has been checked, you must Save in order for the CAMIS system to check for completed requirements and process as Assessment Complete.
B. When you are finished with all DLR/CPS Investigative Assessment tabs, check Assessment Complete. The system will present edits for each item that is not yet completed according to Assessment requirements.

2. The General Browse Option will be displayed. Scroll down to view the entire DLR/CPS Investigative Assessment.

UPDATE DLR/CPS INVESTIGATIVE ASSESSMENT
Rules for Updating DLR/CPS Investigative Assessments
1. Only the creator of the DLR/CPS Investigative Assessment or a supervisor can update the assessment.
2. Supervisors can reassign an assessment if it has not been marked Assessment Complete. This is done by the supervisor sending a new worker ID to the non-completed assessment, and should be done only when a different worker must complete it.
3. DLR/CPS staff can make changes to the assessments after Assessment Complete has been checked only if the Supervisor unchecks the Assessment Complete.
4. The supervisor can also make changes after he or she has unchecked Assessment Complete.

The DLR/CPS Investigative
Assessment is permanently locked
once the Supervisor checks Supervisor
Approval of Assessment. If
permanently locked, the assessment
cannot be changed, except in rare
circumstances, then contact your
CAMIS Trainer.
Instructions for Update
1. Click Open, select Assessment, type the DLR/CPS Investigative Assessment ID.
2. Click OK.
3. Make any changes necessary, unless the Assessment Complete has been checked.
ACTION LOGS
1. Click Browse, select Assessment, type DLR/CPS Investigative Assessment ID. Click OK.
2. Click the Action Logs from the Options list.
3. To print the Action Logs, click Print, select Screen Print or Action Logs for DLR Investigative Assessment ID: e.g. 12200.
PRINT DLR/CPS INVESTIGATIVE ASSESSMENT
1. Select Screen Print if you want to have a print out of exactly what is on your screen.

If you want the complete printed
report, click Print and
select DLR/CPS Investigative
Assessment.
PRINT SUBJECT AND VICTIM NOTIFICATION LETTERS
1. In order to print Notification Letters, the Supervisor Approval of Assessment must be checked by the supervisor. The 45 Day letter can be printed at any time.
2. Once the Supervisor Approval of Assessment has been checked, right click on the subject's or victim's name and select Print, select the Letter (e.g. DLR/CPS Investigative Assessment) you wish to print.

a Print Preview available.
Select the Print
button, select the letter to print.
The CAMIS system will automatically
display the Print Preview.

the left and right arrows to navigate
to additional pages. Click Close
to quit previewing the letter.
4. You must click the Print button (upper left hand corner) on the Print Preview window to print the letter.
5. The Print dialog box will be displayed, you can select another printer or page ranges from this screen. If no changes, click Ok.

(The following reports require Supervisor Approval of Assessment be checked prior to printing, except the last.)
8. You can only select letters that have black text, you can not select ghosted text.
9. When you print Notification Letters, the CAMIS system automatically prints office mailing address and phone number on the letterhead. The supervisor's name and title will go into the closing of the letter. This information comes from your Worker ID.
10. Special Instructions - Letters for Victims
A. To print the victim letters, right-click on the victim's name on the General tab, then select one of the following: Letter for Victim re: Licensed Facility; Letter for Victim re: Unlicensed Facility; Letter for Victim re: State Operated or Certified Facility.
B. You will see the Alternative Address for Victim dialogue box.
C. If the address of the parent or guardian who will receive this letter is different the child's, input that address in the dialogue box. Address 1 field is for physical address. Address 2 field is for P.O Box. Click Ok. This is the address that will print in the Victim Letter.
D. If the address of the parent or guardian is the same as the address of the child then simply click Ok. The system will automatically print the child's address from the child's Person card.

1. Click Tools, click Settings, select the Assessment Module.

A. Assessment
i. Matrix Grouping - This will affect the Matrix tab information on Reassessments, Reunification, Investigative and DLR/CPS Investigative Assessments. By Characteristics - This is the default system setting.
a. By Characteristic - If you check this, it will list the Characteristic in the first column and then by Person IDs in rows.
b. By Person - If you check this, it will list the Person IDs in the first column and then by Characteristics in rows.
ii. Auto-Size Grid Row Heights. When Auto-size is checked, the data entry grids for the Risk Matrix will automatically expand when you enter text. If not checked, then the grid rows will stay the same height.
iii. Prompt for supervisor ID. When Prompt For supervisor ID is checked, request to Print CPS Findings Letters, DLR/CPS Findings Letters and DLR/CPS 45 Day Letter will bring up a dialog box enabling user to change supervisor ID for the requested letter.
Type the supervisor's Office and ID number, click Ok.
