Brief Description
The Reunification Assessment is required when children are returning home after being in placement--due to CA/N--for longer than 60 days. It is based on the person ID, and needs to be done for each child returning home. The Reunification Assessment is recommended when any child returns home from placement under any other circumstances as well.
GUI CAMIS allows the worker to create one Reunification Assessment on multiple children by selecting all applicable children related to the case through a wizard. The worker then completes the child risk matrix on each child, the caregiver risk matrix on each caregiver, the family risk matrix, the overall level of risk, and three questions.
(The GUI CAMIS Reunification Assessment replaces the CPT form on cases requiring CPT review and is intended to be used as an attachment to the ISSP.)

You must select a child's Person ID that is currently in placement.
1. Right click on Child's Person ID from Worker Cases Options, select Send (e.g. SMITH, JAN [2498]) to a new, select Reunification Assessment.

BOILERPLATE INFORMATION
1. Reunification Assessment Boilerplate Information:

A. Assessment ID: The system will automatically assign an Assessment ID once you have saved.
B. Worker: This is the current SSPS Worker ID based on Worker's logon ID.
C. Assessment Date: This is the actual date the Assessment was done. Click on the drop down arrow to select a date or type the Assessment Input Date. The default date is none is select is the Current Date.
i. Click on arrow left/right to scroll through months, click the specific date or click Today to use Today's date or Clear to select another date.
D. Child ID: You can type a Child ID or paste a Child ID. You can also right click search for Person ID.
E. Select Wizard: The Selection Wizard is technically not part of the Boilerplate Information, but is physically located on the Boilerplate area.
1. Check the Select box next to the Case ID(s) of the caregiver(s) you would like to include on this Reunification Assessment.
2. The Selection
Wizard will appear automatically if the child is currently in Placement
and is under 18 years of age only.

3. Click Next to continue.
REUNIFICATION ASSESSMENT WIZARD - SELECT CAREGIVERS
1. Check the Select box next to the Caregivers that you would like to include on this Reunification Assessment.

2. Click Next to continue.
REUNIFICATION ASSESSMENT WIZARD - SELECT CHILDREN
1. Check the Select box next to the Children that you would like to include on this Reunification Assessment.

2. Click Finish to end the Selection Wizard and continue.
3. You may get the following Warning message, if you have selected a child that in not currently in placement.

4. Click Ok and select another child.
5. You may get the following Error message, if you have selected a child that has been marked admin.

6. Click Ok and select another child.
GENERAL TAB
1. General Tab will display the Person ID, Person Name, Date of Birth, Age, Relationship, Primary Caregiver (PC), Case ID and Folder Name information you have provided from the Selection Wizard.

CHILD MATRIX TAB
1. Child Characteristic 1 (Vulnerability/Self protective skills) To see more information on definitions for Risk Factors, click here. The level of susceptibility to CA/N related injury, and/or emotional harm demonstrated by the child based on age, developmental delays, social maturity and past victimization. The child's ability to take action on their own is an important form of self protection.
2. Child Characteristic 2 (Special needs/Behavior problems) To see more information on definitions for Risk Factors, click here. The specialized care required by a child to maintain life, health, safety and well being, including child's behavior which cause challenges for caregivers, schools, and helping professionals.
3. Select a Risk Level appropriate for each Child Characteristic.
0 - No Risk
1 - Low Risk
2 - Moderately Low Risk
3 - Moderate Risk
4 - Moderately High Risk
5 - High Risk
9 - Insufficient Information - requires an explanation.

· Have the parent(s) demonstrated a capacity to consistently meet this child's needs?
· To what degree have the factors that impaired parental capacity at the time of removal been remedied?

Note: Remember to click Save periodically to save your text.
4. Recommendation Regarding Reunification:
If recommendation is for return home, a transition plan is required for children ages eleven years and younger. The recommendation to return home should be done in conjunction with a service plan outlining the parents' participation in services.
Once you have entered some text for Recommendation Regarding Reunification, the Spell check icon on the tool bar is available. You do not have to highlight text. Click Spell check, it will check the text field where your cursor is currently located.
ASSESSMENT COMPLETE
1. Click the Assessment Complete box located on the bottom, right, corner of the screen when all risk matrix questions are complete.

2. Once you check Assessment Complete and click Save, the Selection Wizard is ghosted.
3. In order to change
any information from the Selection Wizard, you must uncheck the Assessment Complete
and resave. Once you have saved, the Selection Wizard becomes available again.
TOOLS - SETTINGS (APPLICATION SETTINGS)
These application settings allows you to customize the view so that CAMIS remembers that preference.
1. Click Tools, select Settings.
2. Click the + (plus sign) next to Modules, click Assessments.

A. Matrix Grouping, the assessment matrix questions can be grouped in two different ways:
i. Matrix questions can be organized By Characteristic, so that each risk question is asked and answered for each individual listed in the assessment. Select by clicking the radio button next to By Characteristic.
ii. Or the matrix questions can be organized By Person, so that all the matrix questions are asked and answered for one person at a time. Select by clicking the radio button next to By Person.
B. Auto Size Grid Row Height
When Auto-sizing is turned on the data entry grids for the Risk Matrix will automatically expand when entering text. If not the grid will rows will stay the same height. Check the box if you want automatic expanding text boxes.
3. Click Apply or Ok to continue or Cancel to cancel making any application settings.
BROWSE FOR AN EXISTING REUNIFICATION ASSESSMENT
1. For a review of an assessment.
PRINT ASSESSMENTA. From the Toolbar
i. Click Browse, Select Assessment.
ii. Type the Assessment ID.
iii. Click Ok.
B. By Person
i. Click Search.
ii. Select Person.
iii. Type Person last and first name.
iv. Select Person sex, click Search.
v. Right click on the Person ID, select Browse.
vi. Click All Assessments list
vii. Click the Assessment ID.
C. By Case
i. Click Browse button, click Case.
ii. Type Case ID (Family Binder "0" number)
iii. Click Ok, click All Assessment listD. From History icon
- Captures the last 10 assessment completed.
i. Click History, select Assessment, select Reunification.

A. Select one of the two following;
i. Select Print Single Reunification, to print the reunification assessment for the child who persons id was first selected
ii. Select Print All Reunifications, to generate a report of all children selected for the assessment.
iii. To print a single Reunification of a specific child, select that child's name on the General tab, then select Print Single Reunification.
B. From the Print dialog box, click Ok.
2. Click here If you need to have a blank Reunification Assessment Form
3. Once you have
opened up this form in MS Word, click File, click Save As, save
this to a location on your hard drive.
TRANSITION AND SAFETY PLAN
A Transition and Safety Plan is required when a Reunification Assessment is positive for the return home of a chld who is age 11 or younger, and has been placed out of the home for 60 days or longer.
Transition and Safety Plan Module
