Brief Description

Transition and Safety Plan is to be completed following a positive reunification decision on the Reunification Assessment. It is required for all children, ages eleven and younger, returning to a parent following 60 days or longer in out-of-home care. For other children and youth, developing a transition and safety plan is encouraged. The chld for whom the Transition and Safety Plan is created must have had a CAMIS placement at some time. CAMIS does not require an Open Placement for a Transition and Safety Plan.

TRANSITION & SAFETY PLAN BOILER PLATE

Assessment ID:This will be automatically assigned by CAMIS as soon as you have saved your TSP.
Assessment Input Date: This is the date the Assessment was input into CAMIS.
Date of Plan: This is the Date the Plan is to begin.
Child ID: This is the Person ID number of a child for whom the Transition and Safety Plan is created.
Child: The name of the child for whom this Transition and Safety Plan is created.

Target Date for Return: to be input by the worker, according to the Reunification plan.

Worker: Your Login name will be displayed here as the Worker.
Case: Name of parent, with whom the child is reunifying, will be displayed.
Related Reunification: This will display the Reunification Assessment Related to this Transition and Safety Plan.
Date Plan to be Reviewed: This is the future date the Safety Plan will be Reviewed.

Selection Wizard is a button that will appear in the Boiler Plate area, but is not technically considered the Boiler Plate. See Selection Wizard section.

 

SELECTION WIZARD

Select Case Wizard

1. Select the Case ID number of the parent with whom the child is reunifying.

2. Click Next.

1. Select one Reunification Assessment that you would like to relate to this Transition and Safety Plan if applicable. If there is no Reunification Assessment, click Next.

2. Click Next.

Select Transition and Safety Plan for Copy Purposes Wizard

Select Transition & Safety Plan for Copy Purposes.

1. To copy text from an existing Transition and Safety Plan of one child to this Transition and Safety Plan, select the existing plan. The text will appear in this Transition and Safety Plan and can be edited, deleted, or have new text added.

2. If not, click Finish.

 

INSTRUCTION TAB

A Transition and Safety Plan is to be completed following a positive reunification decision on the Reunification Assessment. It is required for all children, ages eleven and younger, returning to a parent following 60 days or longer in out-of-home care. For other children and youth, developing a transition and safety plan is encouraged.

The Transition and Safety Plan sets forth the tasks to be completed before and after reunification occurs in order to ensure the safety and well being of the child. This requires that certain basic services are set up for the child in his/her new home, that visitation is designed to facilitate a smooth and careful transition, and that a plan is in place to monitor the child's safety. Additionally, this plan is designed to build upon information outlined in the Kidscreen Evaluation Results - Staffing and Action Plan so that the plan developed while the child has been in care may continue once the child returns home. Services that are contained in the ISSP need not be repeated in this document.

 

SAFETY PLAN FOR THE CHILD TAB

Safety Plan tab.

1. Press the F2 key on your keyboard when your cursor is in the Safety Need, Safety Plan, By Whom or By When grid, a full text editor window will be displayed. You can input up to 1,900 characters of text.

2. Type your text for each of the fields listed below.

3. Click Spell Picture of spell check button.check to check for misspelling.

4. Click Save when done. The text editor window will close.

5. To add another Safety Need, just right click in the grid and select Insert Safety Need or press Enter key when cursor is in the By When column.

6. To delete a Safety Need, click the Safety Need to be deleted, right click in the grid and select Delete Safety Need.

 

MEDICAL/DENTAL TAB

1. Medical and Dental sections are text fields. Type the appropriate information. As the text increases in these fields, the text meter in the bottom left corner will indicate when you have no more room to enter text. If the bar is in the green, you can continue to add text. As it becomes full, it will turn to yellow, and eventually turns red. When it turns red, you can not add more text. Picture of text meter.

2. Scroll bars will automatically be displayed when the text field becomes full. The scroll bars are directly linked to how much text is typed.
3. If you cursor is in the Medical field, you can access Spell check, by clicking the Spell Picture of Spell check button.icon. You must first click your cursor in the field you want to check for spelling errors.

4. Medical Field

Who will be the new medical provider, does the child require an EPSDT be completed? Type the new medical provider name.

How will medical records be transferred to the new provider? (Include information on Optometrist if appropriate.) Type a description of the records transfer.

5. Dental Field

Who will be the new dental provider? Type the new dental provider name.

How will dental records be transferred to the new provider? Type a description of the records transfer.

 

EMOTIONAL/SCHOOL TAB

Type the appropriate text in the fields listed below.

1. Emotional/Behavioral Field

If there is a diagnosed mental health condition, how will it be addressed? If there is a behavioral management plan, how will that be
carried out? How will counseling, if any, be continued? And finally, if there are any other social service needs, how they be met?

2. School Field

Where will the child attend day care or school? Has the child been enrolled? Have records been requested for transfer?

 

HOUSING/FINANCE TAB

Type the appropriate text in the fields listed below.

1. Housing Field

Is the parent's housing safe and sufficient to meet the family needs? Has the family been referred for housing assistance?

2. Finance Field

Has the parent been referred to a financial worker to examine for what assistance the family will be eligible? Families can add a child to their grant 30 days prior to reunification.

VISITATION/OTHER TAB


Type the appropriate text in the fields listed below.

1. Visitation Field

What is the visitation plan before the child is reunified with the parent? Does the visitation plan allow for visits increasing in length and unsupervised? How will the visitation plan be adjusted to meet the emotional needs of the child?

2. Other Field

What other arrangements does the parent believe are needed in order to safely transition the child home?

 

REQUIREMENTS TO SAVE

The following things are required in order to Save a Transition and Safety Plan.

 

ASSESSMENT COMPLETE

The following things are required in order to check Assessment Complete.

1. A child Person ID must be entered.

2. A Date of Plan must be selected and entered.

3. The Wizard must be run, and at least one Case ID must be selected.

4. A Target Date for Return must be selected and entered.

5. A date selected and entered for Date Plan to be Reviewed.

6. If text exists in any one of the following text areas under Safety Plan for the Child, then at least one character of text needs to be present in all of the text areas:

     

UPDATE TRANSITION AND SAFETY PLAN

1. Click Open, select Assessment, type the Transition and Safety Plan ID.

2. You can update the Transition and Safety Plan by the following actions:

3. If you change any information associated with the Selection Wizard, you will see this Confirm message, "Current Transition and Safety Plan Assessment data will be replaced. Do you want to continue"? The data that will be replaced may be the following: Case ID - displayed on boilerplate, Related Reunification - displayed on boilerplate, or copies text from another Transition and Safety Plan to this one. These items are replaced only if you change them in the wizard. Any other text does not get replaced

4. If you click Yes, it will replace all of the information with the modified information entered.

5. If you click No, it will not replace the original information.

 

PRINT TRANSITION AND SAFETY PLAN

1. Click Print.

2. Select Screen Print is you wish to print exactly what you see on your screen.

3. Select Transition and Safety Plan is you wish to print a report that displays all of the Transition and Safety Plan information:
