Brief Description
Information from the completed Foster Care Rate Plan form is entered into CAMIS which then automatically calculates the Foster Care Payment Rate for the child. CAMIS generates a Foster Care Payment Plan showing the payment amount to the Foster Parent(s) for the SSPS authorization period.

ANATOMY OF FOSTER CARE RATE PLAN ASSESSMENT
1. The Boilerplate contains the following fields:
2. The Worksheet needs to be filled out by the Foster Care Worker and Foster Parent prior to the Foster Care Worker completing the Foster Care Rate Plan in CAMIS. The worksheet contains the Social Worker SSPS ID number, the number of children placed with teen parent, Assessment Questions, View Plan button and the Calculate Rate button.
3. The Assessment Tab Grid will display the Child Information, Rate Information, Foster Information and Assessment Information. It is displayed in the left side of the Foster Care Rate window.
ASSESSMENT TAB

The Assessment Tab will display the following fields:
INSTRUCTIONS TAB
1. The Instructions tab serves as an interactive field Help. Move the cursor, without clicking anything, over the Foster Rate Worksheet fields. In the graphic below, the cursor is placed over the Education. The instructions for that field will be displayed in the Instructions tab grid, see orange outline in graphic below.

Note: If you wish to copy any of the Instructions, actually click on the field (Worksheet field) you wish to copy instructions for. This will stop the instructions from roaming as your cursor moves.
BROWSE WORKER CASESA. Click and highlight the portion(s) of text you wish to copy.
B. Click Edit, select Copy (Ctrl + C), select Paste (Ctrl + V) in your document.
Browse Worker Cases
1. Click Browse, select Worker. The Worker ID is associated with the Login ID. The default is set to the Login person's Worker ID. You can browse for another Worker, by typing in their Worker ID.

2. Click OK to use the default Worker ID or type a Worker ID.
3. Click Worker
Cases from the Options on the left side of your screen. A list of open assignments will
be displayed. Right click on the Person ID from the list of Open Worker
Assignments to Cases.
4. Select Send (Person ID e.g. 140826) to a new, select Foster Rate.
Browse by Worker, Foster Care Records
1. You can also click on the Foster Care Rates Options to view the Worker's Foster Care Records.

2. Right click on any Foster Rate ID and select Browse Foster Rate, select General, Foster Rate Agreement, Worksheet or Action Log.
or
3. Right click on the Child Name (that has a closed Plan End Date) and select Send Person (ie 5312) to a New, select Foster Rate.
CALCULATE RATE UNRELATED TO CHILD
1. To determine a Calculation rate unrelated to a child for the Foster Care Rate based only on the information entered. Rates calculated from the Calculation only can not be saved.

2. Click New, select Foster Rate.
A. Check the Calculation Only box.
B. Click the down arrow to select the date, for the Child Birth Date.
C. Enter the Start Date.
D. Enter the End Date.
E. Answer the Assessment Questions 1 - 6.
F. Click Calculate Rate.
i. This will calculate a rate based on the Child's Birth Date and assessment answers.
ii. The Assessment tab on the left, will display the Foster Care Rate Information.
CREATE NEW FOSTER CARE RATE PLAN
1. Click the New
icon, select Foster Rate.
2. Type a child's Person ID.
3. The Foster Care Rate Wizard button will be available once a Child ID is entered.
4. Type the child's Person ID with an associated Placement or Legal Custody record. Once a Person ID (with Placement or Legal Custody) is entered, click the Foster Care Rate Wizard button. The Foster Care Rate Wizard button is ghosted until a Person ID (with Placement or Legal Custody) is entered.
FOSTER CARE RATE WIZARD - PLACEMENT EVENT(S)

1. Check the appropriate Placement event in the Select column for the new Foster Care Rate Plan for the appropriate time period. Placement Events will be displayed in this wizard based on what Person ID was used to initiate the Foster Care Rate Plan.
2. Click Next.
FOSTER CARE RATE WIZARD - PLAN DATES

To read the Valid License Types, hover your cursor over each Valid License Type codes.
1. Select new Foster Care Rate Plan Date for child with the selected Placement Event.
A. Plan Start Date - Type the date or click the drop down arrow to pick a date from the calendar, that the plan will start. The plan start date can to be more than 30 days from the foster rate create date. E.g. It's July 1, 2004 - you can not create a new Foster Care Rate August 1, 2004. It must be within 30 days. This will default to one of the following:
i. If no foster rate has been completed, the start date begins 1/1/2001.
ii. If a foster rate plan has been completed, the start date begins the next day of the plan period. E.g. A plan ends Jan 15, 2001, the start date of the new plan is Jan 16, 2001.
iii. If there is a gap between 2 existing foster rate plans, the start date and end dates fall between the 2 plans. E.g. Foster rate plan A has a start date of 1/1/2001 and an end date of 3/15/2001. Foster rate plan B has a start date of 6/1/2001 and an end date of 12/31/2001. A new plan date created today will show a start date of 3/16/2001 and an end date of 5/31/2001.
B. Plan End Date - Type the date or click the drop down arrow to pick a date from the calendar, that the plan will end. This will default to 6 months past the start date. You will be able to change the Plan End Date after the Placement Event has ended.
- If a new Placement Event has been created, you may need to change the existing Plan End Date before you create a new Plan Start Date.
- Note: If child disrupts or leaves the placement prior to the Plan End Date, you must update the Foster Rate Plan.
2. When you have selected a child over the age of 22 and have clicked Finish in the Wizard, you will see this message displayed. The child must be younger than 22 to be used in the Foster Care Rate Plan. Click OK and check the child's Person ID to make sure it is the correct Person ID and select a child under the age of 22.

3. Click Finish.
FOSTER CARE RATE - WIZARD (OUT OF STATE)
1. When you use a child's Person ID that is Out of State (OS), the Plan Dates Wizard screen is the only screen displayed.
2. The only fields you can modify are the Plan Start Date and Plan End Date.
3. Plan Start Date must be later than the OS Legal Custody Date. Payment can not be made if a child is not in the Legal Custody of Children's Administration or another state.
4. Click Finish.
VERIFY ASSESSMENT INFORMATION
1. Review and verify the Assessment Information, if anything is not correct e.g. the Primary, Secondary, Date of Licensure, etc. you will need to go into BUSRELS in Classic CAMIS. In BUSRELS, change any Business ID information and save your changes. Come back to the Foster Care Rate and rerun the Foster Care Rates Wizard.
WORKSHEET
The Worksheet needs to be filled out by the Social Worker and Foster Care Parent prior to the Social Worker completing the Foster Care Rate Plan in CAMIS. The worksheet contains the Social Worker SSPS ID number and the number of children place with teen parent.
Responses to the following Assessment Questions should be based on the average number of hours per week for the next 6 months using what you know of the child's needs and the foster parent's ability to provide for those needs. All questions reflect activities consistent with normal parental expectations and skills. The foster parent may not be able to meet all of the child's needs and therefore answers should reflect the amount of time that the foster parent actually expects to spend in each area with the individual child.
While responding to these questions the foster parent should consider the level of care they will provide to this child compared to the level of care they would provide to a typically developing child of the same age.
The Assessment Questions are in the Worksheet. The worksheet is what the Worker and Foster Parent fill out prior to the Worker completing this Foster Care Rate Assessment in CAMIS. Answer each of the Assessment Questions, click the drop down arrow to make your selection.
Assessment Questions
1. Care of Child - What is the average number of hours per week the Foster Parent(s) will need to concentrate specifically in caring and /or advocating for and supervising this child for the following reasons?
A. Physical Needs - What is the average number of hours per week the Foster Parent(s) will need to spend providing the level of parental care, support and supervision necessary to meet the needs of the child due to child illness or special physical conditions?

WORKSHEET - SOCIAL WORKER SSPS ID & # OF ADDITIONAL CHILDREN
Once you have answered all of the questions on the Foster Care Rate Wizard screens and click Save or Calculate Rate, the system will create an Assessment screen.
1. Social Worker SSPS ID number is based on the user's Login ID. Enter the child's current assigned SSPS Worker ID.
2. Type the # of additional children placed with a teen parent in the same Foster Home. Only enter # of children when child of teen parent is not dependent. Use the up and down arrows to the right of (1) to increase and decrease this number.
WORKSHEET - CALCULATE RATE
1. Click the Calculate
Rate
button to determine the rate, this will display the Rate Information on
the Assessment grid to the left.
2. The Calculate Rate button is ghosted until all required fields are completed.
3. If you make a change in the Worksheet Questions, the Calculate Rate button is available to select again.
Medicaid Personal Care Service (MPC)
A. The first time you click Calculate Rate, you will get this Information Message.
B. Read carefully and consider if the child has additional medical considerations.
i. If the child has additional medical considerations, click Comprehensive Assessment (MPC) DSHS Form 14-396. This form applies for additional services from MAA.
ii. Save the form to a location on your computer.
iii. Complete the form.
iv. Contact your SSPS Coordinator for your Region for further assistance in authorizing MPC (SSPS Manual, Appendix C 4900 series).
C. Click OK on the Information message.
WORKSHEET - VIEW PLAN
1. The View Plan button is ghosted until you have Saved the Foster Rate. Once the Foster Rate is saved, the View Plan is available.
2. Click View Plan
button. This creates an Adobe PDF file of the Foster Rate Agreement. You can preview the plan before you print it.
3. You do not need to save and store these PDF files, they will be saved and stored within the CAMIS GUI Foster Care Rate module.
WORKSHEET - FOSTER RATE FORM
1. This link will open up a blank Foster Care Rate Assessment DSHS 10-261 Form. Print this form and keep for using when away from the office.
1. Click Open, select Foster Rate, select either Person ID, Foster Rate ID or Business ID.
A. Open By Person ID
i. If you have selected Open Foster Rate by the Person ID, type the Person ID, click OK.

ii. There will be a list of Foster Rates related to the Person ID you have entered. From the list, click on the Foster Rate and click Open.
iv. Once the Foster Rate opens, you will be able to do one of two things:
a. If within 7 days of creating the Foster Care Rate, you will be able to edit the entire Foster Care Rate Agreement.
b. If after 7 days, the Foster Care Rate Assessment is locked, you will not be able to edit the Worksheet. You will only be able to Open, Rerun Wizard (you can only change the Plan Start and End dates), View Plan, and Print the Foster Care Rate Agreement.
B. Open by Foster Rate ID
i. If you have selected Open Foster Rate by the Foster Rate ID, type the Foster Rate ID, click OK.
ii. This will Open the Foster Rate Assessment. Once the Foster Rate opens, you will be able to do one of two things:
a. If within 7 days of creating the Foster Care Rate, you will be able to edit the Worksheet. You will only be able to Open, Modify, Save, View Plan and Print the Foster Care Rate Agreement.
b. If after 7 days, the Foster Care Rate Assessment is locked, you will not be able to edit the Worksheet. You will only be able to Open, View Plan and Print the Foster Care Rate Agreement.
C. Open by Business ID
i. If you have selected Open Foster Rate by the Business ID, type the Business ID, click OK.
ii. There will be a list of Foster Rates related to the Business ID you have entered. From the list, click on the Foster Rate and click Open.
iii. If you have selected Open Foster Rate by the Business ID, type the Business ID, click OK.
iv. This will Open the Foster Rate Assessment. Once the Foster Rate opens, you will be able to do one of two things:
a. If within 7 days of creating the Foster Care Rate, you will be able to edit the Worksheet. You will only be able to Open, Modify, Save, View Plan and Print the Foster Care Rate Agreement.
b. If after 7 days, the Foster Care Rate Assessment is locked, you will not be able to edit the Worksheet. You will only be able to Open, View Plan and Print the Foster Care Rate Agreement.
BROWSE FOSTER RATE BY PERSON ID
1. Click Browse, select Foster Rate, select Person ID.
2. Type the Person ID, click OK.
3. When the Browse window is displayed, you can click on any of the hyperlinked text to review that information in more detail. All related Foster Rates will be displayed for the Person ID.
4. Information will not be displayed on the Browse screen unless you have ran the Foster Care Rate Wizard and have saved the Foster Care Rate.
3. Right click on the Foster Rate ID in the left column under Foster Care Records, select Browse Foster Rate e.g. 637, then select one of the following Options:
4. Once a Browse is displayed, you can click any of the Options on the left to toggle between Browse pages. This will be displayed when you Browse by Person ID, Foster Rate ID or by Business ID.

5. Right click on any of the hyperlinked fields in the grid area to view, browse, open, relate or send options.
6. If the Foster Care Rate record has been created before May 24, 2004, the Placement Start Date and Placement End Date will not be displayed. The Placement Start Date and Placement End Date will be displayed if created after May 24, 2004 or if a Foster Care Rate record has had the Foster Care Rate Wizard rerun and resaved after May 24, 2004.
BROWSE FOSTER RATE BY FOSTER RATE ID
1. Click Browse, select Foster Rate, select Foster Rate ID .

2. Type the Foster Rate ID, click OK.
3. Once a Browse is displayed, you can click any of the hyperlinked Options on the left to toggle between Browse pages. This will be displayed when you Browse by Person ID, Foster Rate ID or by Business ID.
4. Right click on any of the hyperlinked fields in the grid area to view, browse, open, relate or send options.
BROWSE FOSTER RATE BY BUS ID
1. Click Browse, select Foster Rate, select Bus ID.
2. Type the Business ID, click OK.

3. Right click on the Foster Rate ID, select Browse Foster Rate e.g. 564, select General, Foster Rate Agreement, Worksheet or Action Log. You can also right click and select Open Foster Rate e.g. 564.
4. You can also click the Business Facility Complaints from the Options list on the left.
PRINT FOSTER RATE AGREEMENT
Click Foster Care Rate Assessment DSHS 10-261 form to open and print a blank form.
1. Open a Foster Rate Agreement.
2. Click the Print
icon.
3. Select either Screen Print, Print Foster Rate Agreement, or Preview Foster Rate Agreement.
4. The Foster Rate Agreement report will display the following information:
5. Print a copy of the completed Foster Rate Agreement and give to the Foster Parent(s) when a placement or re-authorization occurs.
