Brief Description
A Service Episode Record (SER) tracks the documentation of actions taken by Social Work staff on a Referral, Case, License, Facility Complaint or Person. CAMIS Policy & Procedures #25 - Service Episode Records.

SEARCH SER BY LOGON ID
1. Click Search, select Service Episode Record.

2.
Type the Logon ID.
3. SER Input Date
A. Click the drop down arrow to select a Start Date.
B. Click the drop down arrow to select an End Date. Maximum of 45 days between Start and End date for search.
4. Click the drop down arrow to select an Activity.
5. Check Quick Search (No name information will be displayed)
A. Unchecking Quick Search should be done with reservation, this may take several minutes to perform.
B. Use Quick Search for short time frames. (45 days or less between Start and End date.)
C. Select an Activity.
D. The Quick Search is checked by default.
6.
Number of SERs found: (0-how
many SERs match the criteria)
CREATE A NEW SER
A. Click Browse,
select Worker. The Worker ID is associated with the Login ID. The default is set to the Login person's Worker ID. You can browse for another Worker, by typing in their Worker ID.
B. Click OK to use the default Worker ID or type a Worker ID.C. Click Worker Cases Option on the left side of your screen. If you are a Licensor, click Worker Licenses or Worker Facility Complaints. A list of open assignments will be displayed. Right click on the Case ID from the list of Open Worker Assignments to Cases.
Note: These Case IDs are hyperlinked to more information. To browse more details about the case, referral, license, person or complaint ID, click on the case ID for that item.
D. Select Send (Case ID#) to a new, select Service Episode Record.

E. You can also click Worker Referrals or Worker Licenses, right click on Case ID from the list of Worker Referrals/Licenses, select Send (Case ID#) to a new, select Service Episode Record.

Note: If you select Send to versus Send (Case ID#) to a new you must first open a new SER, then the ID will be sent to the SER.

2. An SER must have these things in order to be saved.
3. Activity Date. Click on the down arrow to access the calendar. To save an SER, you must enter the date of the activity. You can type in the date or click on the drop down arrow next to the calendar box.
A. Activities that are TCM, Date Sensitive (Date the activity occurred), Data Management, are bolded.
B. Click the down arrow to access the calendar window. You can scroll to previous or future months with the left and right arrows. Click once to select the date.

4. Time. Use the up and down arrows to select the Time. The time is not required, but is recommended, especially if the user wants multiple SERs with the same activity date to print in chronological order. Use 24-hour time.
5. Send SER Alerts - If this box is checked, it will send an alert to the assigned worker when you are writing an SER on a case, referral, license or etc, that is not assigned to any other assigned worker except the creator. This alert will be emailed to the assigned worker through MS Outlook and will advise them that an SER has been written on their case (referral, license, etc) and suggest that they review the new SER.
6. New SER button. This creates a new, duplicate SER with the same Referral and Case (Relationship) information. The SER text will not be carried over into new SER, however, you can copy and paste text as needed. Be careful that the original SER text, if being copied, still pertains to the relationships in the new SER text.
7. Targeted Case Management (TCM) Questions
A. If you select the activity, Child - Face to Face With Child, you will be asked this TCM question, "Is the child in placement?" Click Yes or No. Your answer will be displayed in a read-only field.
B. Or if you select the activity, Child - Telephone Call, you will be asked this TCM question, "Was this contact lasting approximately 15 minutes or longer relating to the assisting a child "IN PLACEMENT" in gaining access to needed medical, educational, social or other services?" Click Yes or No. Your answer will be displayed in a read-only field.
C. If you select Child - Face to Face with Child or Child - Telephone Call, you will be required to relate a Child's Person ID (under age 22) to this SER.
D. If either Child is in Placement or Call duration was at least 15-minutes. The TCM boxes are checked.
E. Exception to CAMIS GUI Standards: Bold text indicates date sensitive activities including Targeted Case Management & Data Management.
8. Location
A. Once you have selected at least one SER Activity the Location field becomes available.
- It is required to select a Location if these Activities (Health and Safety Monitoring Visit or Child - Initial Face to Face with Child) on the DCFS CPS tab are checked.
- It is enabled for all other activities except Child - Telephone Contact.
B. Click on the drop down arrow next to Location, select the Location.
A. This is a number automatically generated by CAMIS once the SER is saved.
10. Type
A. This is automatically generated by the system, based on the Activity tab you have selected. These are codes only used for reporting and troubleshooting.
11. Input Date/Time
SEARCH FOR SERA. The date and time worker input SER information.
Quick Search

1. Check Quick Search to quickly see a list of your SERs by Logon ID.
2. No name information will be displayed, and ID # will be displayed.
3. Type your Logon ID (300 ID).
4. Use short time frames. SER Input Date has a maximum of 180 days for search.
5. Select an activity.
6. Unchecking the Quick Search should be done reservation. Unchecking Quick Search will take some time to return all of the search results.
7. Click Search.
There are ten different Wizard screens, they are all similar and function the same way. You can add the Case, Referral, Person, License, and Facility Complaint information from any of these wizards in any sequence. There is no set order in which you have to select a Relationship Wizard.
1. Click on the Tab (Cases, Referral, Person, License, and Facility Complaint) and type the ID associated with that tab. E.g. If you click Cases, you'll need to type a Case ID.

3. Sorting & Moving Column Headings within the Wizard screen.
A. The Related Person(s) columns are sortable and the column headings are movable. Just click on the heading, drag and place it where you want it to be. Notice the arrow to the right of the Heading title. The arrow indicates the ascending or descending order, click to change the order.
B. When you are in the appropriate area to move the column heading you will see two green arrows, indicating the new placement. (See below graphic, in this example, the Person ID is moved over one column to the right).
C. Click Next.
4. Grouping and Sorting Columns
You
can select columns to group and
sort by, simply drag a column
heading to the dark gray area
to sort by that column. Move that
column to the far left to have
that column appear first. This
allows you to only preview the
information from the column headings
you've selected in a drill down
method. In the example below,
the first drill down column heading
is Related Case, then Last Name
then by First Name.
Note:
The column heading
must stay within the column heading
area or just above the darker
gray area. This symbol
will warn you that you can not
move into any other location on
this grid.
5. Wizard Screens
RELATIONSHIPS TABA. Family Case(s) Related to the Referral Wizard
i. Check the Select box of the Family Case ID Related to the Referral.
ii. Click Next.
B. Select Referral(s) Related to Person(s) Wizard
i. Check the Select box next to the Referral ID of the Referral(s) Related to the Person(s).
ii. Click Finish.
C. Select Person(s) Related to Referral(s) Wizard
i. Check the Select box next to the Person(s) ID Related to the Referral(s).
ii. Click Next.
D. Select Facility Complaint Related to Referral(s) Wizard

ii. Click Finish.
Note:All DCFS SERs are to be written on the Parent Case number (not the child's ID) or the referral unless the child is legally free, on a Guardianship order, in the care over age 18, or on an ICPC child. If the child is legally free or in a guardianship (this must be documented in the Legal and Placement module in CAMIS), over age 18 or ICPC, then send the child's Person ID to the SER.
* Right click on any ID to Browse the information or Send Related Ids to the SER.
1. Cases Sub Tab (This is a list of all of the Case ID(s) that are related to the SER.)
A. Click the Cases tab, type the Case ID, press Enter. You can then use the Related to Cases Wizard.
2. Referrals Sub Tab (This is a list of all of the Referrals related to the SER.)
A. Click the Referrals tab, type the Referral ID, press Enter. You can then use the Related to Referrals Wizard.
3. Persons Sub Tab (This is a list of all of the Persons related to the SER.)
A. Click the Persons tab, type the Person ID, press Enter. You can then use the Related to Persons Wizard.
4. License Sub Tab (This is a list of all of the Licenses related to the SER.) License and Facility Compliant are only available if the SER Type is MDLRCPS.
A. Type the License Control ID if known.
B. If the License Control ID is not known, right click in License Control ID field, select Search, select License.
C. You can perform a Search by Facility Name, by City, by Zip Code or by County.
i. Search for License by Name
a. Type the Facility Name or part of Facility Name. You do not have to type a City, Zip or County to perform a search.
b. Click Search or Clear to type another Name.
c. Once the search is complete, look in bottom left hand corner for Search Complete. You can group and sort the column headings, click here for detailed instructions on how to rearrange column headings. If you have moved column headings around and want to restore, click Reset Display.
d. You can also save these search results in an Excel spreadsheet. Click To Excel. The only way to retrieve this spreadsheet is to Save As, give it an unique file name and save to a location on your Network drive.
e. Right click on Business ID from the search results list, select Send License e.g. 59844 to, select e.g. PRE GUI SER 568065 - License.
f. Close the search window by clicking the (-) minus in the gray window.
ii. Search By City
a. Type the City Name
b. Select a Facility Type from the drop down list, this is not required to perform a search. See list below for Types of Facilities

a. Type Zip Code.
b. Select Facility Type from drop down list.
c. Click Search.
d. Once the search is complete, look in bottom left hand corner for Search Complete. You can group and sort the column headings, click here for detailed instructions on how to rearrange column headings. If you have moved column headings around and want to restore, click Reset Display.
e. You can also save these search results in an Excel spreadsheet. Click To Excel. The only way to retrieve this spreadsheet is to Save As, give it an unique file name and save to a location on your Network drive.
f. Right click on Facility Name, select Send License e.g. 16725 to, select SER e.g. 568065 for Referral ADAMS, STACY - License.
g. Close the search window.
iv. Search By County
a. Select a County Name from the drop down list.
b. Select Facility Type from drop down list.
c. Click Search.
d. Once the search is complete, look in bottom left hand corner for Search Complete. You can group and sort the column headings, click here for detailed instructions on how to rearrange column headings. If you have moved column headings around and want to restore, click Reset Display.
e. You can also save these search results in an Excel spreadsheet. Click To Excel. The only way to retrieve this spreadsheet is to Save As, give it an unique file name and save to a location on your Network drive.
f. Right click on Facility Name, select Send License e.g. 56 to, select SER e.g. 568065 for Referral ADAMS, STACY - License.
g. Close the search window.
5. Facility Complaint Sub Tab (This is a list of all Facility Complaints related to the SER.) License and Facility Compliant are only available if the SER Type is MDLRCPS.
TEXTA. Type the Facility Complaint ID if known. Click Search.
B. If unknown, right click in the Facility Complaint ID field, select Search, select Referral.
C. From this Search By Date Referral Search window, type the following information.
i. Type or use the drop down arrow to pick a Received Start date.
ii. Type or use the drop down arrow to pick a Received End date.
iii. Check Complaints or Non Complaints.
iv. Check all Program Codes that apply.
v. Check all Decision Codes that apply.
vi. Check all Risk Tags that apply.
vii. Click Search.
D. Right click on Referral ID, select Send Referral e.g. 1330963 to, select SER e.g. 568065 for Referral ADAMS, STACY - Referral.
E. Close the search window.
1. You can start typing your narrative before you type any other information in your SER. See the Text Tab.
SER ACTIVITIES - DCFS TABA. Watch the colored bar in the bottom left corner. The color bar will start out green. When the color bar turns yellow you are near the end of your typing space. When the color bar turns to red, you have filled the maximum space allowed for text. STOP! There are only 105 lines of text available.
B. If you need additional space for text, create a new SER with the time one-minute later.
i. End the first SER text with "Continued in Next SER".
ii. Begin the next SER with "Continued from Prior SER".

TEXT TAB
1. You can select Text tab to start typing your text before you enter any other information. Click the Text tab, click your mouse in the text field and start typing.
2. Here are a few things to remember about SER Text.

3. Every time text is added, it records the user, date and time.

Once you have typed your text,
click
to check spelling.

If you need more room to see your
New text or your Original text,
move your cursor to the bottom
of the white New Text box. Your
cursor will turn into a
two horizontal lines with up and
down, when you hold right at this
line. Now move the cursor up or
down to increase/decrease the
box sizes.
6. If you need additional room for text, create a new SER with the time one-minute later.
A. End the first SER text with "Continued to Next SER".
B. Begin the next SER with "Continued from Prior SER".
SAVE SER
1.
In order to Save
your SER, you must have the following:

BROWSE AN EXISTING SER
1. If the SER ID is known, click Open, select Service Episode Record, select SER ID.

A. Type the SER ID, or other ID if known.
B. click OK.
2. If the SER ID or other ID is Not known, click Browse, select Worker, select Worker Cases. From the list of Open Worker Assignments to Cases, right click on Case ID, select Browse (Case ID), select SERs.

Note:
Browse the Case, Referral,
etc. itself, without going
through Worker Cases, this
saves a few steps.

3. From the list of SERs, select the SER ID. Right click on SER ID, select Open (SER ID).
1. The SER is locked after 7 calendar days of the SER Activity Date.
|
Note:
If you are creating an SER
for an activity that occurred
7 or more days ago, be sure
the text is complete and
correct before you click
Save. The SER will
lock immediately upon saving
and you will not be able
to make changes to the original
text. You will have to use
the Add Text button.
|
2. If you need to edit the original SER, open the SER.
A. Click Text tab, then click Add Text. You cannot change the original text on a locked SER. The Add Text tab allows the original worker or another worker to add text. The added text will attach itself to the original text and show the User ID who typed it, the date and the text.
B. The worker who created the SER can change the date, activities, and related ID numbers even when the SER is locked. Simply open the locked SER and choose the correct date or activity, then Save your changes. Contact your CAMIS trainer for help.
3. If you need to delete an incorrect Case, Referral, Person ID and send to the right ID, follow the directions below.
A. While the SER is open, click the Relationships Tab, click Cases.
i. Right click on Case ID, select Delete Case (e.g. 99D0044444).
ii. A delete Confirmation message will display. "Are you sure you want to delete Case e.g. 8787?" Answer Yes, No, Cancel, All - All for multiple Cases related to SER.
iii. Click in the Case ID field, type in the correct Case ID if known.
Note: You can right click anywhere on the SER, search for the Person that the SER is about, look at cases related to the Person and send the appropriate Case to the SER.iv. If you do not know the Case ID, search for correct Case ID.
a. Click Browse, select Case.
b. Type Case ID number.
c. Click SERs from the Options on the left.
d. Right click on SER ID.
e. Related to [SER ID Number - e.g. 22411].
f. Select Cases.
g. From the Cases related to Service episode record [e.g. 22411] screen.
h. Right click on Case ID.
i. Select Send Case e.g. 92L0107710 to.
j. Select SER for Case Last Name, First Name - Case.v. This will relate the recently selected Case ID to the open SER.
TO CHANGE THE REFERRAL ID
B. While the SER is open, click the Relationships Tab. Click Referral.
i. Right click on Referral ID, select Delete Referral (e.g. 8787).
ii. A delete Confirmation message will display, "Are you sure you want to delete Referral e.g. 8787?" Answer Yes, No, Cancel, All - All for multiple referrals related to SER.
iii. To search for correct Referral ID, Search the Person, look at related Referrals, and send the right Referral(s).
iv. This will relate the recently selected Referral ID to the open SER.
TO CHANGE THE PERSON ID
C. While the SER is open, click the Relationships Tab. Click Persons.
i. Right click on Person ID, select Delete Person (e.g. 4295).
ii. A delete Confirmation message will display, "Are you sure you want to delete Last Name, First Name (e.g. 4295)?" Answer Yes, No, Cancel, All - All for multiple Persons related to SER.
iii. To search for correct Person ID.
a. Right click Person ID field on the Persons tab.
b. Select Search.
c. Select Person.
d. Search Last Name and First Name.
e. Click Search.
f. Right click on Person ID.
g. Select Send Person ID e.g. 4295 to
h. Select SER Case Last Name, First Name - Person
v. This will relate the recently selected Person ID to the open SER.
If you have created an SER prior to the U-M Changes, you will see this in the blue title area of the SER, "SER ID: e.g. 24478, Created Prior to U-M Changes."

1. There are only three things you can do with a Created Prior to U-M Changes;
PRINT SERA. Add additional text. Click the Text Tab, click Add Text. This will bring up a text editor, type the text, click Spell check if needed and click Save.
B. Click Relationship Tab, you can view the Cases, Referrals, Persons, Licenses, Facility Complaints.
C. Click New SER. This will create a new SER with the existing Cases, Referrals, Person, Licenses, or Facility Complaints information.
1. There are two ways of printing SERs. The first way to print is if you know the SER ID. You can print only one SER at a time with this method. Click Open, select Service Episode Record, select SER ID. Type the SER ID, click OK. Select Print, you then have two choices; Screen Print or Print SER.

A. Screen Print will print a picture of what you are seeing on your screen. It is in essence, a snapshot of your active window.
B. Print SER will print a report of the following;
- Activity Type
- Persons Related to SER
- Referrals Related to SER
- Cases Related to SER
- License Information Related to SER
- Facility Complaints Related to SER
- SER Text
2. The second way to print will also allow you to View Text before you print, and gives you several options for printing SERs, including filters for date range and type of activity.
A. Click Open, select Service Episode Record, select one of the following;
- SER ID
- Case ID
- Referral ID
- Person ID
- Facility Complaint ID
- Ref. ID (Facility Complaint)
B. Type the (SER, Case, Referral, Person, Referral, etc ID)C. Click OK to continue.
D. The title bar will say "SERs for Case ID e.g. 29D1054460". This ID will change based on which type of ID you selected on the previous screen. In this example, SER for Case ID was selected.
E. Select your SER IDs in two ways;
i. Click on one SER ID.
ii. Press and hold the Ctrl key down and select multiple SER IDs not in a row.

F. There are four buttons on the bottom of the screen: Open, Browse, View Text and Cancel.
i. Open - This will open each SER ID and display the SER Text. If you have selected more than one SER ID, close each window after viewing the information.
ii. Browse - This will allow you to Browse quickly each SER Text. Toggle between SER by clicking on the SER ID in the upper left area. The text will display in the SER Text area to the right.
iii. View Text - Select one SER ID, click View Text. If there is any SER Text, it will display in a text box. Click OK to continue.
- Tip: Selecting more than one SER ID will make it hard to tell which text belongs to which SER ID. The Text box will not display the SER ID or the Case, Referral, etc. name.
iv. Cancel - This will cancel all actions and take you back to a blank CAMIS screen.
G. Click on the SER ID. Click Print. The following are the options for Printing:
i. Screen Print - This will print what you see on the screen.
ii. Print SER - This will generate an SER History report for the type of ID you've selected. (e.g. Case ID 29D1054460). Click OK in the Print dialog box.
iii. Print All SERs - A warning message will display, click OK. Click OK in the Print dialog box. This will print an SER History of every SER ID related to the Case ID, e.g. 29D1054460.
a. Select a Start Date.
b. An End Date is automatically defaulted to the current date, but may be changed.
c. Check the appropriate action(s).
d. Click OK to continue.
v. Print AAG SERs - This will allow you to print Selection Criteria, limited to Action Code(s) AG and limited activity dates that you have specified.
a. Select a Start Date.
b. An End Date is automatically defaulted to the current date.
c. Click OK to continue.
vi. Print Preview SER - This will allow you to preview an SER, you can then print an SER History report for the (Case ID e.g. 29D1054460). Click OK in the Print dialog box.
vii. Print Preview All SERs
a. This will allow you to preview all SERs by the following criteria:
- SER ID
- Activity Date
- User Name, Location Code
- Activity Type
- Person, Case, Folder Name, Referral, License, Facility Complaint
- SER Text
viii. Print Preview SER Options - This is the same as number (iv.) above, however this will allow you to Preview the SERs before you Print. Click Print, this will generate an SER History report with the options you have selected.
ix. Print Preview AAG SERs
a. Select a Start Date.
b. An End Date is automatically defaulted to the current date.
c. Click OK to continue.
d. Preview the selected AAG SER information, you can then click Print and print that SER History for that ID.
1. Click Tools, select Settings, select SER Module.

2. When Automatically Invoke Relationship Wizard is checked, the Relationship Wizards will automatically start up when a valid ID is entered.

| Legally Free | A legally free child can not be placed on an SER with a child who is not legally free; however, two legally free children can be on 1 SER. Example: Visit between two legally free siblings. |
| Parent Case ID | The Parent Case Number begins with a two digit office code , then a letter, then a seven digit case number ending in 0. (Ex: 92L0001230) |
| Pre-UM | Before Union Management Committee imposed new rules. |
| Referrer | Someone who contacts DCFS with a concern. |
| SER | Service Episode Record |
| TCM | Targeted Case Management |
